How to Qualify

Qualification for the Michigan Medical Marijuana Program is an easy process once you have contacted a Medical Marijuana Card Doctor. While we must follow Michigan law, we still make the medical marijuana application process a quick and easy one. Following are the steps in the qualification process.

Before anything, you must have a medical condition that qualifies you for the use of medical marijuana under Michigan Laws and Statutes. Refer back to our Getting Certified page for Qualifying Conditions.

Medical Marijuana Card Doctors offer free per-screening for people who may qualify, however a visit with one of our physicians, which is required to complete the application process is not included in the free consultation. The qualifying patient should have their most recent medical records available for the doctor to look at during this appointment. These medical records should include pathology reports, MRI’s, X-ray’s and/or prescription documentation.

For most patients we require that they have an existing primary care physician. If you currently do not have a primary care physician and are over the age of 40, ask about our chronic pain program. Your visit with our physician will be limited to discussing your case as it applies to your application for a Michigan Medical Marijuana Identification Card.

Step One - Medical Records:

The first step in the process is  documentation of your medical condition. Without current  medical records, our physicians will have a difficult time qualifying your condition, however if you have prescriptions or old medical records those can help too.

Step Two - Certification Appointment:

Our physicians will talk with you about your qualification for certification of a medical condition that may qualify you for a Michigan Medical Marijuana Card. By law, we can only discuss your qualification for the program. After  approving your condition, qualified patients will receive a signed Physician Certification Form that will allow them to apply to the state for a Medical Marijuana Identification Card. In order to receive the Physician Certification Form, patients must present a valid Michigan ID and pay the physician visit fee which is $79.

Step Three - Application for your Michigan Medical Marijuana Card:

In order to receive your Medical Marijuana Identification Card, you must send the following information to the Michigan Medical Marihuana Program:

» Physician’s Certification
» Proof of Michigan Residency
» $60 Application Fee

»» $85 Application Fee if applying with a caregiver
»» If Applying with Caregiver also provide proof of their identity

The information you provide will be verified within 21 days of receiving the application. If approved, your Medical Marijuana Identification Card will be issued and sent to the address you provided within 2-6 weeks.
